[At-review] Revised staff questions from Becky and Brian

Dear Team members
I attach a next draft of the questions, and seek agreement to send  
these to Rod.

I have added the question asked by Larry, numbered the questions ( a  
very hot topic with me is numbering paragraphs of docs. under  
discussion), and redlined the changes.

I have added no new content.

The question under "Board governance" is actually about two separate  
topics; governance, and appeal mechanisms.
I have separated them out.

I have also separated out the individual questions asked about para  
9.1, and which are said to apply to each of the individual topics  
that follow.

Please let me have any further thoughts. Thanks to Becky and Brian  
for the hard yards.
