[IOT] Updated Rule 4 text

I had an action to produce a clean copy of the Rule 4 text on the repose and safety valve, to reflect our provisional agreement.  I also offered to propose revised language following our discussion on our last call that terms "exceptional" or "extraordinary" have specific legal meanings in some jurisdictions which might not reflect our intent.  Since our last call there had also been a proposed revision from Malcolm on clause H, which I have retained as a comment in order that the group can review this.  Finally, I have included a note to this draft Rule 4 that the time for filing is subject to the concept of the Fixed Additional Time which we have agreed.
