[Ws2-diversity] Call for consensus questionnaire / Deadline Friday 3rd 12:00pm UTC

since the deadline already passed and only comments suggesting small
changes were shared, we can consider the questionnaire as agreed by the
subgroup. please find attached the last version to be sent for next week
plenary.
