[council] Form for 2012 budget requests

Dear Council members,

Please find attached the form that should be used for budget requests for Fiscal Year 2012 (a "pdf" version was embedded in the presentation that was circulated in December).  Forms should be completed by the SO/AC leadership (the Chairs of the SGs and Cs as well as the Council itself if there are budget items that the Council chooses to request) and sent to: controller at icann.org<mailto:controller at icann.org>.

The deadline is the end of January 2011.
