[NPOC-EC] Members Survey

Dear Caleb and all, I think is good as is. However, I would added a question about the name of organization member and their Representative for NPOC in order to updating and contrasting our database to know what organizations are themselves consider as NPOC members. 
I have heard from you that we have some phantom members and I think would be a good opportunity to being revealed.
