[Area 4] Merging Areas 2 and 4

I think that before we conclude on
"identify[ing] the main contingencies that CCWG Accountability will use
to test the proposed mechanisms and solutions, once they are elaborated"

we should compare the list we have against comments reviewed by Area 2,
to ensure that there weren't things identified there that we ought to
include here.

I've had a first go with some Area 2 items I'm more familiar with. As a
result I've added three items and generalised one existing one; see
attached. This isn't suggested as being complete.
