Hi Ariel,

thanks for the draft survey.
I can't seem to add comments to the survey, so I'll just write my 
comment to the list (though it'd be more convenient if we could comment 
directly).

In Q1 you write:
[IF ANSWER IS NO, PROCEED TO Q5]
[IF ANSWER IS DON’T KNOW / NOT SURE, PROCEED TO Q5]

However, there does not seem to be a Q5 if I haven't missed it. Or is 
this the standard way to denote that the questionnaire ends in this case?
Or maybe you wanted to put "PROCEED TO Q4" instead. After all, I think 
it makes sense to ask the question "What factors may affect your 
decision to request activating allocatable variant label(s)?" also to 
those people that answered "no" or "don't know" to the first question.
